Fever, vomiting, infection... and Diabetes?

Sick Day Rule

• Check your sugar every 4–6 hours. Sugar levels can change rapidly when you are sick.

• Do not stop Diabetes medicines on your own. Medications like Metformin, DPP4 inhibitors (Teneligliptin / Sitagliptin), and insulin usually need to be continued.

• Some medicines may need to be stopped temporarily during dehydration caused by vomiting, diarrhea, or reduced fluid intake. For example, Dapagliflozin should be paused temporarily.

• Increase your intake of water and fluids. Take small sips frequently to avoid dehydration.

• Do not remain completely fasting even if you cannot eat regular meals. Soups, yogurt, thin khichdi, and coconut water are acceptable.

• If you are on insulin, it usually needs to be continued. Sugar often rises during illness.

• Consider urine or blood ketone testing, especially if sugar is very high.

• Contact your doctor if you experience any of the following symptoms: - Sugar consistently above 250 mg/dL - Persistent vomiting - Severe weakness or confusion - Inability to drink fluids

• Do not delay hospital evaluation in severe illness as it may be necessary.
